Because patient’s lives change throughout the year, another determination of eligibility will take place at the end of each year. The information provided on these forms will be used by GCFC to determine your eligibility for medical benefits and on average will take sixty (60) minutes to complete. This includes the time it will take to read instructions, gather necessary facts and fill out the forms.
