Determining Your Eligibility

  •      On the patients first (1st) visit they will go through an intake process to
    determine if they are eligible for services through the Gratiot County Free
    Clinic.  Because patient’s lives change throughout the year, another
    determination of eligibility will take place at the end of each year.  The information provided on these forms will be used by GCFC to determine your eligibility for medical benefits and on average will take sixty (60) minutes to complete. This includes the time it will take to read instructions, gather necessary facts and fill out the forms.

         On the first (1st) visit the person must provide the following eligibility documentation:

    Residency i
    n Gratiot County:  In order to prove this the
    person must provide one of the following: 
    • driver's license, or
    • state issued identification (ID) card, or
    • letter with current address, or
    • a utility bill with current address.

    Proof of Age:
      In order to receive services the adult must be eighteen (18) years of age or older. In order to prove this the person must provide one of the following:
     
    • driver's license, or
    • state issued identification (ID) card, or
    • copy of birth certificate.

    Proof of Income: In order to prove this the person must provide one of the
    following
    • Copy of Federal Income Tax return (most current), or 
    • Social Security Income Benefit, or
    • Most current month's
      paystub for the past two (2) months 

     Federal Poverty Guideline: For those who cannot afford medical care, and whose income is at or below the two hundred and twenty five percent (225%) of the Federal Poverty Guideline. In order to prove this the person must provide one of the following:
    • Copy of Federal Income Tax return (most current), or 
    • Social Security Income Benefit, or
    • Most current month's paystub for the past two (2) months 

    Not Eligible When You. . .

    When you. . .
    • Live outside Gratiot County
    • Have commercial insurance or are eligible for Government Health Plans including County health plans, Medicaid, Medicare or MI Child
    • Qualify for Veteran’s medical insurance
    • Have a work-related injuries involving workers compensation
    • Have refused to apply for government insurance or provide proof of
      income upon the request of the clinic
    • Are intoxicated or abusive
